High Adventure Trip 2008 Forms

• Travel Itinerary.  Day by day plan of activities.
   
• Packing Checklist.  What to bring on the trip.
   
• Permission Slip.  This form must be notarized.  A notary will be available at the scout house on Friday night, May 9.  If you are not available this evening, please make arrangements for your own notary before May 9.  (Many banks offer free notary services.)  This form must be completed by May 9, 2008 in order for a scout to participate in this summer's high adventure trip to Banff Canada.
   
• Class 3 Medical Form.  To be completed by your family physician.  This form must be completed by May 9, 2008 in order for a scout to participate in this summer's high adventure trip to Banff Canada.
  
  
• Rafting Waiver. This form must be completed by May 9, 2008 in order for a scout to participate in this summer's high adventure trip to Banff Canada.


• Passport. (This is a sample)  Bring your passport for photocopying by May 9, 2008.
   
• Health Insurance Card. (This is a sample)  Bring you health card by May 9, 2008.
