High Adventure Trip 2008 Forms

The following forms must be completed by May 9, 2008 in order for a scout to participate in this summer's high adventure trip to Banff Canada.

• Permission Slip.  This form must be notarized.  A notary will be available at the scout house on Friday night, May 9.  If you are not available this evening, please make arrangements for your own notary before May 9.  (Many banks offer free notary services.)
   
• Class 3 Medical Form.  To be completed by your family physician.
  
  
• Rafting Waiver. 


• Passport. (This is a sample)
   
• Health Insurance Card. (This is a sample)
